coolant and cap

my radiator cap wont come off my 88 ranger 2.9 i untwisted it all the way and i still wont come off how? Also how much coolant do i put in like 2 inches from the top or something??
The last 1/8 turn you have to push down while turning.
As far as how much coolant, well I just fill it up till it spills out. Any over that and it will go to the overflow container.
