1988 Bronco ii problems accelerating and ratteling noise

Well I wake up every morning and before I leave for school I turn my car on 5 min before I leave and when i switch from park to reverse or to any other gear it pulls/jerks. And when I start driving it makes this loud ratteling noise and goes on for a couple mins till it warms up. Next problem is that when I accelerate its really slow. It will start like at 1-2 rpm then jump to 4 rpms after a couple secs. I know its not normal. I've changed the oil and spark plugs and wires and the fuel filter. Those are some fixes that I have done. This is a 1988 ford bronco ii 2wd automatic with 57 miles on it(it rolled over from 99,999) please let me know what can be done. If i can do it also cause money is tight now n day. Thanks!!!
